Solar Elipse Opt/In Form and Release of Liability

On Monday, August 21, 2017, a solar eclipse will be visible, weather permitting, across all of North America.  A form has been sent home to you via your child for you to grant (opt-in/check "yes" box) or not grant (mark "no" box) permission for your child to participate in outdoor activities with special eclipse glasses.  Your child may not go outdoors during the eclipse if you do not return the permission form with the "yes" box checked.
